Culturally Responsive Pharmacy Care in Population Health Practice: A Case Study
CHALLENGE
A Black woman faced communication gaps, provider indifference, and a lack of clarity around her medications. The result: disengagement, poor adherence, and missed outcomes.
STRATEGY & INSIGHT
A Black pharmacist working at a community-centered pharmacy identified the breakdown in care by simply listening to the patient. She did more than just explain medications; she took time to ask about health goals, concerns, and barriers. What's more? She has lived experience as a Black woman. With this insight, the pharmacist understood the importance of creating space for the patient to share on her own terms.
In this encounter, the pharmacist:

Slowed down the medication review process to reduce confusion.

Honored the patient’s experience and cultural context.

Rebuilt confidence in care by explaining options with empathy.
Even the environment and culturally resonant music played a role in reestablishing emotional safety. These intentional, culturally competent touchpoints helped transform a routine medication consult into a catalyst for engagement, while reinforcing trust in the pharmacist and in pharmacy care and preventing an avoidable emergency visit.
IMPACT
Research shows that culturally concordant care, such as this, increases adherence, satisfaction, and health outcomes across chronic conditions. Culturally competent interventions do more than show respect; they demonstrate effectiveness.
This is why pharmacy training must reflect the lived realities of Black communities, because Black pharmacists are already solving real problems. Let’s equip more to do the same.

Culturally Competent Pharmacy Training for Healthcare Organizations
Partner with aPHP for population health pharmacy consulting and equity-aligned workforce strategy. We support healthcare organizations in addressing disparities in Black communities through culturally competent pharmacy practice and system-level workforce transformation.
To operationalize this work, we collaborate with HR leaders, compliance teams, and talent managers to design pharmacist development programs grounded in cultural competency and structural humility, aligned with ACPE standards and organizational performance priorities. Our flexible virtual model equips pharmacy teams to strengthen retention, elevate workforce capability, and advance measurable quality and equity goals for historically excluded populations.
